We have imaging locations from the far East side of Ohio to the far West side and throughout all of Ohio including Dayton, Columbus and Youngstown.

Columbus Area Locations

There are 3 convenient locations in the Columbus region. This includes the Jasonway location that is centrally located just west of Rt. 315 off of Bethel Road; the Gahanna locations which are northeast in Gahanna and northwest in Dublin. Our new Dublin Cleveland Clinic Imaging location is the only outpatient facility in the Columbus metro area with a 3 Tesla MRI, which provides the highest levels of image quality currently available.

Appointments

Many of our locations have different phone numbers to make appointments. A referral from your physician is required to make an appointment.

Registration

Please bring your insurance card and driver's license with you to the appointment, and please arrive 15 minutes before your scheduled appointment. It is necessary that the Imaging staff has your written order for the exam from your physician. Please make sure this has been faxed to our office or bring it with you at the time of your exam. Once you arrive at the Cleveland Clinic Imaging facility you will be asked to complete a small amount of paperwork, which includes the following:

Registration form.
Acknowledgment of Receipt of Notice of Privacy Practices.
Screening form.
Acceptance of Financial Responsibility.
Patient Acknowledgement and Consent for Billing Insurance.
Authorization for Release of Medical Information.
